What is a PCN

​ A Primary Care Network (PCN) is a collaborative group of general practices working together in a local area to deliver coordinated health services. PCNs were introduced in 2019 as part of the NHS Long Term Plan to improve the delivery of primary care and to provide a broader range of services to patients. 

About Five Elms PCN 

Formed in 2019, Five Elms Primary Care Network (PCN) is a collaboration of five GP Surgeries in Bromley, Biggin Hill, and Orpington.

We pride ourselves on offering our patients the best care possible by working in collaboration with our allied health care professionals across the network.

Five Elms PCN offers and supports in-house training of Nurses, Physician Associates, and GPs, and, as an educational hub for health care professionals where we can offer the best and most proactive forms of learning .
