About Five Elms PCN
Formed in 2019, Five Elms Primary Care Network (PCN) is a collaboration of five GP Surgeries in Bromley, Biggin Hill, and Locksbottom, Orpington.
We pride ourselves on offering our patients the best care possible by working in collaboration with our allied health care professionals across the network.
Five Elms PCN offers and supports in-house training of Nurses, Physician Associates, and GPs, and, as an educational hub for health care professionals, can offer the best and most proactive forms of learning .
By working together we are able to offer our services at times that may be more flexible for our patients across the PCN.

General Practitioners
Five Elms PCN has a wide and diverse mix of highly experienced General Practitioners that all share the same aspiration to provide the best possible care to their patients. All our GP's involve and collaborate in multidisciplinary team work including nursing and other allied healthcare professionals in the care of their patients

Nurse Specialists
We have an expansive range of experienced Nurses with various skills and expertise. The majority of our Nurses have over 10 years’ experience in Practice Nursing. Some specialise in chronic disease management such as Diabetes, COPD and Asthma. All our Nurses are fully equipped in running day to day treatment room duties.
Clinical Pharmacists
Our clinical pharmacists are responsible for reviewing medication and dealing with medication queries. They work very closely with prescription clerks at the practices, pharmacists in medicines management and optimisation as well as liaising with community pharmacists to ensure that high-quality care and safe prescribing is maintained.
Social Prescribers
We currently have two Social Prescribers working across our network providing valuable social care to their patients. Both Social Prescribers are a key element of our multi-disciplinary team (MDT), who ensure your healthcare is managed holistically.
Care Coordinators
Five Elms PCN has 2 Care Coordinators who are responsible for bridging the gap between patients and healthcare professionals to organise a personalised care plan centred around what matters to you.
Nursing Associates
We are currently training a Nursing Associate who is undergoing University education and practical placements within a range of settings across South East London.
Paramedics
The PCN has two Paramedics from the London Ambulance Service working as one whole time equivalent across the network and spends equal times in our 5 member Practices. An additional Paramedic is being recruited at the moment with a view to establishing a team of 3 over the next 12 months.
Advanced Care Practitioner
The PCN has recruited an ACP who has joined the team - Advanced Clinical Practitioners (ACP) come from a variety of professional backgrounds including nursing, physiotherapy, pharmacy, paramedics and occupational therapy. They are experienced healthcare professionals with a Masters level award or equivalent who have developed their skills and knowledge to take on expanded roles and scopes of patient care.
Mental Health Practitioner
Five Elms PCN has now employed two Mental Health Practitioners. They work closely with other PCN based roles to help address the potential range of biopsychosocial needs of patients with mental health problems e.g., with clinical pharmacists for medication reviews, and social prescribing link workers for access to community-based support.
Physiotherapist
There are a number of Advanced and First contact Physiotherapists working across the Five Elms Network. Appointments are available every Saturday and occasional weekdays
Health & Wellbeing Coach
We have hired a new Health and Wellbeing Coach. They work with patients to help them achieve personal, fitness and lifestyle goals. Our coach provides personalised lifestyle support, helping patients to increase activity levels, improve eating habits and maintain the long-term changes they need to improve their health and wellbeing.
