Our Services
Health & Wellbeing Cafe
Five Elms PCN now run 3 wellbeing cafés every month. One being at St Augustine's Church in Southborough Lane, Bromley, another at St Marks Church in Biggin Hill and now a third one in St Pauls Church, Crofton Road, Orpington.
All patients aged 65 and over are welcome to attend, where they can come and enjoy a cup of tea and biscuits, whilst listening to talks from external organisations and clinical professionals. Patients can also have their blood pressure taken and get involved in the various activities such as Love to Move and Bingo.

At our Cafes we provide a safe place for people to come and socialise and get out into the community.

St Pauls Church on Crofton Road, Orpington is the first Tuesday of every month, 11am-1pm. No booking required.
St Marks Church at Biggin Hill is the second Friday of every month, 10am-12pm. No booking required.
St Augustine's Church on Southborough Lane is the last Friday of every month, 11am-1pm. No booking required.
Enhanced Access
As part of the Enhanced Access programme Five Elms PCN offers a mix of pre-booked telephone, face to face and online appointments every Saturday where patients can access a GP, Nurse, Clinical Pharmacists and Advanced Care Physiotherapist's. We also run remote weekday sessions with the GP and Clinical Pharmacist.

Social Prescribing
Our Social Prescribing Link Workers are the bridge for non-medical issues between your GP and all the non healthcare services available within your community. They can link you to the right service to support you.
They can help:

Connect patients to groups, activities and services in the community.

Support patients with complex needs.

By consulting with you to discuss your situation.

Assess your needs and offer advice around support available to you.

By talking to you about any non-medical issue including debt, loneliness, education, discrimination etc.

Refer you on to community services for further support.

Please speak to your GP about a referral to speak to the social prescribing link worker.

Care Coordinator
The Care Coordinator is responsible for bridging the gap between patient and healthcare professionals, to organise a personalised care plan centred around what matters to you.
​
They can help:

Patient support for home visits and follow up appointments.

Support patients in making shared decisions.

Support carers.
-
Liaise with district nurses regarding dressing changes, blood tests, etc.

Adult social care support for ICN referrals, hospital equipment and safeguarding issues.

Liaise with the occupational therapy for physiotherapy, falls clinic and aids around the home.

Chase hospital appointments, follow ups and arrange hospital transport, pre admission and discharge notifications from the hospital.​

BP@Home
Blood Pressure @Home aims to increase availability and access to blood pressure monitoring for patients with poorly controlled hypertension or patients who have not had their blood pressure checked in the last 5 years.
​
Patients are either supported to use a blood pressure machine they own or can gain access to, or if this is not possible, can have self-book an appointment in their practice to have their blood pressure checked. Patients will receive a BP@Home self-booking link by PCN team.
BP @Home is part of the project to scale up PCN remote monitoring Hub’s being developed in Bromley to:​​​

Enable personalised clinical support to be delivered remotely and help people to better manage their health and care.

Reduce inequalities in health care CVD events due to uncontrolled hypertension.

Provide more convenient, high quality and timely alternatives to face-to-face care, where this works for the individual, and when clinically appropriate.

Provide a sustainable model/approach at PCN level to the management of blood pressure monitoring.
Group Consultations
What are Group Consultations?
​
In traditional clinician-patient appointments, such as asthma or diabetes reviews, sessions typically last 10-15 minutes.
​
In contrast, group consultations extend to around 90 minutes, enabling up to 12 participants to engage more deeply with their healthcare team—and with each other.
​
Sessions are typically led by a facilitator from the practice team, with specialised medical or therapeutic input provided by relevant clinicians or allied health professionals.
​
Group consultations can focus on any condition but particularly LTCs such as Fibromyalgia.
Digital Inclusion HubÂ
For NHS patients, digital inclusion means having easy and affordable access to a suitable device with sufficient data and internet connectivity, and the digital skills and health literacy to use them safely and confidentially to access NHS services.
Promoting the use of NHS app to a targeted audience is part of the Five Elms PCN digital inclusion agenda.

We are currently in the process of opening a Five Elms PCN digital hub. More information to follow as soon as the hubs are up and running in the local community.
