Our Services
Health & Wellbeing Cafe
The wellbeing café is held every 2 weeks alternating between St Augustine's Church in Southborough Lane, Bromley and St Marks Church in Biggin Hill. All patients aged 65 and over are welcome to attend, where they can come and enjoy a cup of tea and biscuits, whilst listening to talks from external organisations and clinical professionals and they can also have there blood pressure taken and get involved in the various activities such as love to move and bingo.

At our Cafes we provide a safe place for people to come and socialise and get out into the community.
​
St Augustine's Church is the last Friday of every month 11am-1pm. No booking required.
St Marks Church is the second Friday of every month 10am-12pm. No booking required.
Enhanced Access
As part of the Enhanced Access programme Five Elms PCN offers a mix of pre-booked telephone, face to face and online appointments every Saturday where patients can access a GP, Nurse, Clinical Pharmacists and Advanced Care Physiotherapist's. We also run remote weekday sessions with the GP and Clinical Pharmacist.

Appointments are pre-bookable through your GP practice.

Social Prescribing
Our Social Prescribing Link Workers are the bridge for non-medical issues between your GP and all the non healthcare services available within your community. They can link you to the right service to support you.
They can help:

To connect patients to groups, activities and services in the community.

Support patients with complex needs.

Your social prescriber will consult with you to discuss your situation.

Assess your needs and offer advice around support available to you.

Talk to your social prescriber about any non-medical issue including debt, loneliness, education, discrimination etc.

They can refer you on to community services for further support.

Please speak to your GP about a referral to speak to the social prescribing link worker.

Care Coordinator
The Care Coordinator is responsible for bridging the gap between patient and healthcare professionals, to organise a personalised care plan centred around what matters to you.
​
They can help:

Patient support for home visits and follow up appointments.

Support patients in making shared decisions

Support for carers
-
Liaise with district nurses regarding dressing changes, blood tests, etc

Adult social care support for ICN referrals, hospital equipment and safeguarding issues

Liaise with the occupational therapy for physiotherapy, falls clinic and aids around the home

Chase hospital appointments, follow ups and arrange hospital transport, pre admission and discharge notifications from hospital.​

Health and Wellbeing Coach
The Health and Wellbeing Coach will work with patients to help them achieve:

Help the patient to set realistic goals

Help the patient to identify the barriers

Supports patients going through bereavement, end of life, menopause and offers cancer support and more

Our Health and Wellbeing Coach provides personalised lifestyle support. They will offer the patient advice on how to increase there activity levels in line with there lifestyle. Offer advice around improving eating habits, and help to maintain the long term changes they may need to improve their overall health and wellbeing.

Digital Inclusion HubÂ
For NHS patients, digital inclusion means having easy and affordable access to a suitable device with sufficient data and internet connectivity, and the digital skills and health literacy to use them safely and confidentially to access NHS services.
Promoting the use of NHS app to a targeted audience is part of the Five Elms PCN digital inclusion agenda.

We are currently in the process of opening a Five Elms PCN digital hub. More information to follow as soon as the hubs are up and running in the local community.
