This form should be used for patients to self-refer to the PCN’s Care Coordinators. The criteria for self referral is:

you must be over the age of 18
registered at one of the five practices outlined (The Bromley Common Practice, Norheads Lane, Southborough Lane Surgery, Stock Hill Medical Centre or Summercroft Surgery)

The service provides assistance in managing and organising patient care by evaluating care delivered by different primary and community care teams. Over the age of 18 can self refer
