Five Elms Primary Care Network was formed in 2019 compromising of 5 GP practices Norheads Lane Surgery, Southborough Lane Surgery, Stock Hill Medical Centre, Summercroft Surgery and The Bromley Common Practice, Our PCN covers a population of 43,500 patients.
We focus on delivering patient-centered services that promote health and wellbeing, working collaboratively to ensure that all individuals have access to the care they need.

Important Announcement: Enhanced Access Clinic Location Change
We would like to inform our patients that from the 19th of April 2025, the Enhanced Access Clinic currently held at The Crown Medical Centre will be relocating to Summercroft Surgery, Orpington BR6 7AR. This relocation is part of our ongoing commitment to improving patient access and service delivery.

Please note that this change applies only to the clinics that were previously held at The Crown Medical Centre.
The Enhanced Access clinic will continue to run on every second Saturday of the month at Stock Hill Medical Centre as usual.
