Group Clinics at Five Elms PCN
Five Elms Primary Care Network (PCN) support five practices in Bromley (The Bromley Common Partnership, Southborough Lane Surgery, Summercroft Surgery, Stock Hill Surgery, Norheads Lane Surgery).

We feel there are benefits to running Group Clinics in both a face-to-face or video group session and evidence shows there are benefits for patients with long term conditions e.g. diabetes, high blood pressure, or even patients living with persistent symptoms such as Menopause or Long COVID.

We are currently running Group Clinics for people with newly diagnosed Reduced Kidney Health. If you fall into this group, there is a good chance you may be offered a group clinic appointment to review your condition.
Benefits to this way of supporting patients​
​
· They are scheduled well in advance, so you get your routine appointment with plenty of notice and can plan around it with no need to ring the surgery
· You get to spend longer with our team, and time to reflect and get answers to your questions
· You get ideas to help you keep well and better manage your health condition or symptoms
· You feel supported and validated because you meet others in the same situation. You feel less alone and better able to cope with your health issues
· You learn more and gain confidence to manage your health issues back at home.

Face-to-face group clinics work well if you are more flexible in terms of time or feel a bit worried about using technology.
​
Video group clinics can be convenient if you work or have mobility issues.
​
You are welcome to bring your carer to the group clinic.
